2010 Make A Difference Application & Agreement For Nonprofit Organization

Please read the following agreement carefully and then submit your online application.

Hereinafter the Nonprofit Organization will be referred to as NPO.

NPO understands that vending is not allowed in the Make a Difference area, unless agreed upon
ahead of time by Summer Camp Nonprofit Coordinator, Jamie Bender.

NPO agrees to have all workshops and/or presentations approved ahead of time by Summer
Camp Nonprofit Coordinator, Jamie Bender.

NPO agrees to provide Summer Camp Nonprofit Coordinator, Jamie Bender with information
about their NPO for the Summer Camp Brochure by April 12, 2010. Any NPO that registers after
this date will not have information on their NPO listed in the brochure.

NPO agrees to provide Summer Camp Nonprofit Coordinator, Jamie Bender with information
about their NPO for the Summer Camp Guide by April 12, 2010.

NPO agrees to inform Summer Camp Nonprofit Coordinator, Jamie Bender by May 10, 2010 if
NPO is no longer attending Summer Camp Music Festival 2010.

NPO will be provided with two (2) passes for the event. If additional passes are needed, they can
be purchased for the full ticket price, unless agreed upon with Summer Camp Nonprofit
Coordinator, Jamie Bender ahead of time.

NPO booth location will be determined by Summer Camp Nonprofit Coordinator, Jamie Bender.

NPO will provide their own table (and tent if desired) no bigger than 10x10ft, along with all other
equipment (furniture, signs, hangings, etc) needed, unless previously arranged with Summer
Camp Nonprofit Coordinator, Jamie Bender.

NPO understands that Summer Camp Music Festival 2010 will not provide, nor be responsible
for NPO's equipment. NPO is responsible for the security of all equipment, product, and personal
effects during the Summer Camp 2010 Music Festival. Promoter will provide overnight security,
however; Promoter takes no responsibility for any losses incurred through theft, carelessness,
force majeure, riots, power outage or power surges.

In order to complete the application, NPO must acknowledge the
following terms and conditions page by reading and signing at the bottom.


TERMS & CONDITIONS FOR ALL NPOs

1. LOAD-IN TIMES: Wednesday, May 26, 2010 from 9am until 8pm or Thursday, May 27, 2010
from 9am until 4pm. If you have a special need, please contact Jamie Bender at 773.791.2014.

2. FESTIVAL DATES & TIMES: Thursday, May 27, 2010 (1 pm - Midnight; pre-party), Friday,
May 28, 2010 (11am - Midnight), Saturday May 29, 2010 (11am - Midnight), Sunday May 30,
2010 (11am - Midnight ) Grounds open to the public on Thursday, May 27, 2010 at noon.

3. EXHIBITION TIMES: Your booth must be manned Friday through Sunday from 10am to
5:30pm unless otherwise arranged with Summer Camp Nonprofit Coordinator, Jamie Bender.
You are also strongly encouraged to have your booth open on Thursday, May 26th. Having your
booth open at other times is optional.

4. FESTIVAL WRISTBANDS: Your festival wristbands are good for the staff/volunteers of YOUR
NPO ONLY. Wristbands cannot be given to those not involved in the NPO's work at Summer
Camp Music Festival 2010.

5. TRASH: NPO shall make an extra effort to oversee all areas immediately surrounding his/her
booth to avoid accumulation of trash and residue. NPOs are required to participate in the
Greening and Waste Management plans by separating their recycling and reducing their overall
waste at the festival. NPO is responsible for any damage to the grounds resulting from NPO's
operations during the Summer Camp 2010 Music Festival. Designated fire lanes must be kept
clear at all times.

6. Absolutely no food, drink, or other products may be sold by NPO, unless previously agreed
upon with Summer Camp Nonprofit Coordinator, Jamie Bender.


ACKNOWLEDGEMENT OF TERMS

I hereby warrant and confirm that the below information is, to the best of my knowledge, true and
correct, and further certify that I have read all of the information provided in this application,
checked the agreement box below in the application form that indicates compliance with all terms
and conditions, and agree with the Terms and Conditions provided.

The NPO hereby indemnifies the Summer Camp Music Festival, Progressive Innovations, Ltd.
dba Jay Goldberg Events & Entertainment, Jam Productions, Ltd., Three Sisters Park, its agents,
employees and offices against and agrees to defend and hold them harmless from any and all
liability, loss, expense, damage, claims and causes of action arising out of or resulting from or in
connection with the NPO's work or activities.
